Parent and Child Questionnaire
Many thanks for offering to assist with our study! The study is open to parents (main carers) and their children, aged between 7 and 11 years.
The questionnaire is about your own eating behaviour, your child’s eating behaviour, the way you encourage your child to eat and your experiences in close relationships. There are also a few questions for your child to answer about his or her relationships with other children. Answering all the questions typically takes 30 minutes. Everyone who completes the questionnaire will be entered into a prize draw to win £50!
- Please note that you will be asked to provide height and weight information for you and your child. Please ensure that this is as accurate and up-to-date as possible. You may wish to weigh yourself and your child just before you begin completing the questionnaire. Please ensure that height and weight measurements are taken without shoes and heavy clothing.
- Please ensure that the child who is participating with you is aged between 7 and 11 years, and that only one child participates per family.
- It is best to complete the questionnaire at a quiet time when you are unlikely to be disturbed. The majority of the questions are to be completed by you, the parent. A small number of questions are to be completed by your child.
- Your child’s part of the questionnaire may be completed separately if that is most convenient. You will be asked to provide your email address so that we can match up your answers with your child’s answers.
- All answers are confidential.

This online questionnaire has been approved by the ethics committee of the Faculty of Science at the University of Bristol (if you have any questions regarding this procedure please contact Ali.Prescott@bristol.ac.uk). Before filling out the questionnaire, please take the time to read the ‘Informed Consent’ below. Proceeding with the questionnaire constitutes acceptance of these statements.
Informed Consent
- I understand the nature of the online questionnaire and that my answers and my child's answers will be used for research purposes only.
- I understand that any information submitted on this website is confidential and will be anonymised before any data analysis takes place (February 2012).
- I agree to the School of Experimental Psychology in the University of Bristol handling and processing this information about me and my child. I understand that the information will only be used for the purpose(s) set out above and my consent is conditional upon the University complying with its duties and obligations under the Data Protection Act 1998.
- I understand that I can withdraw my data and my child's data prior to anonymisation at any time and without having to give a reason by emailing Charlotte.Hardman@bristol.ac.uk and my information and my child's information will be removed immediately.
- I agree to receive the prize draw result and a short report of the findings by email.
